Guest Self-Certification for International and/or Guests
Pursuant to New Hampshire State guidelines for lodging facilities, all registered guests who have traveled internationally or on cruise ship, must provide a driver’s license as proof of residency and attest that they have quarantined for 10 days prior to arrival in New Hampshire / 10 days after their last date of travel.
Quarantine Statement required for guests who have traveled internationally and/or by cruise ship:
By signing this statement, I attest that I and all of the individuals staying with me in my rented home have:
- Quarantined* at home for the past 10 days prior to arrival in New Hampshire unless the travel was to/from Canada for essential purposes.
- Are fully vaccinated and two weeks have passed since their final dose of their vaccine or have had a diagnosed case of COVID-19 within the past 90 days.
- NOTE: If more than 90 days have passed since a positive diagnosis of COVID-19, then this exemption does not apply.
*The definition of quarantine in this instance means only going out for essential items and when outside of the home maintaining social distancing and wearing face masks when within less than six feet of another person during this 10 day “quarantine”.
